Comment Request for Review of Revised Information Collection: Standard Form 2808

The Office of Personnel Management (OPM) is seeking public comment on a revised information collection, Standard Form 2808, which allows individuals covered by the Civil Service Retirement System (CSRS) to designate a beneficiary for a lump sum payment in the event of death. The OPM has submitted the request for review to the Office of Management and Budget (OMB) and is seeking public input on the proposed collection.
