U.S. Department of State Requests Approval for New Visa Page Application Form

The Department of State is seeking OMB approval for a new information collection, the Application for Additional Visa Pages, which will be submitted by approximately 30,000 individuals or households each year, requiring about 10,000 hours of annual response time.
