U.S. Department of State Seeks OMB Approval for Revised Passport Statement Form

The Department of State is requesting a 60-day public comment period for a revised passport statement form, DS-64, which will be submitted to the Office of Management and Budget for approval, following the Paperwork Reduction Act of 1995.
