Department of State Submits 30-Day Notice for Revised Passport Witness Affidavit Form

The Department of State has submitted a request to the Office of Management and Budget for approval of a revised version of the Affidavit of Identifying Witness form, used for passport applications, which is expected to be completed by approximately 140,000 individuals or households.
