Department of Labor Submits Information Collection Request to OMB for Review and Approval

The Department of Labor is seeking public comment on a proposed information collection request for a Health Insurance Claim Form, which is sponsored by the Office of the Workers' Compensation Programs and must be reviewed and approved by the Office of Management and Budget under the Paperwork Reduction Act of 1995.
