U.S. Department of State Requests Approval for Revised Visa Application Form

The Department of State is seeking OMB approval for a revised version of its Form DS-1648, used by aliens applying for A, G, or NATO visas, which is expected to be completed by approximately 20,000 individuals annually, requiring an estimated 10,000 hours of response time.
