Submission for OMB Review Comment Request for Review of an Information Collection

The Office of Personnel Management (OPM) is seeking public comment on a request to review an information collection form used by the Civil Service Retirement System to provide survivor benefits for former spouses. The form, RI 20-64, is completed annually by approximately 30 individuals, taking about 45 minutes to fill out.
