Proposed Collection; Comment Request for Review of a New Information Collection SF 2809-1

The Office of Personnel Management is requesting emergency clearance for a new information collection, SF 2809-1, which will be used by certain Federal retirement systems and former dependents of these individuals. The form, aimed at 9,000 annual completions in 30 minutes each, will be submitted to the Office of Management and Budget for review.
