Agency Information Collection Activities Submission for OMB Review; Comment Request

The Committee for Purchase From People Who Are Blind or Severely Disabled has submitted initial certification forms (Form 401 and Form 402) and annual certification forms (Form 403 and Form 404) to OMB for review and clearance under the provisions of the Paperwork Reduction Act of 1980 (44 USC Chapter 35).
