Submission for OMB Review; Comment Request for Review of a Revised and Expiring Information Collection: Form RI 95-4

The Office of Personnel Management (OPM) has submitted a request to the Office of Management and Budget for review of a revised information collection form, RI 95-4, which is used to pay refunds of retirement contributions for federal employees. The form requires applicants to provide marital information, and OPM estimates it takes approximately 30 minutes to complete annually.
