OPM Submits Revised Information Collection for OMB Review

The Office of Personnel Management is requesting a revised information collection, the Disabled Dependent Questionnaire (RI 30-10), to determine eligibility for health benefits and survivor annuity payments under the Civil Service Retirement System and the Federal Employees Retirement System, with approximately 2,500 forms completed annually.
