U.S. Department of State Requests Approval for Collection of Death Report Form

The Department of State is seeking OMB approval to reinstate a previously approved collection for the Report of the Death of an American Citizen abroad, which will be submitted by survivors, relatives, and estates of deceased American citizens who have died abroad.
