Submission for OMB Emergency Clearance of the Revised Information Collections: OPM 2809-EZ1 & OPM 2809-EZ2

The Office of Personnel Management (OPM) has submitted a request for emergency clearance of two revised information collections, OPM 2809-EZ1 and OPM 2809-EZ2, to the Office of Management and Budget. These forms are used by annuitants, survivor annuitants, and former spouses of retirees to request enrollment changes and informational materials during Open Season.
