Office of Personnel Management Submits Request for Clearance of New Information Collection Form SF 2817

The Office of Personnel Management is seeking Office of Management and Budget clearance for a new form, SF 2817, used by federal employees and retirees to enroll or change their group life insurance elections, which can now be assigned to others
