Submission for OMB Review; Comment Request for Review of a Revised Information Collection: Standard Form 3112

The Office of Personnel Management (OPM) has submitted a request for review of a revised information collection, Standard Form 3112, to the Office of Management and Budget. The form, used for disability retirement applications, will be completed by applicants and their supervisors, with approximately 12,100 forms submitted annually.
