Nuclear Regulatory Commission Requests OMB Review for Medical Examination Certification Form

The Nuclear Regulatory Commission has submitted a proposal to the Office of Management and Budget for review, requesting an extension of its medical examination certification form, NRC Form 396, which is required for facility licensees applying for operator licenses and for renewals of operator and senior operator licenses.
