Medicare and Medicaid Programs; Revisions to Conditions of Participation for Hospitals

The final rule establishes a discharge planning process for Medicare and Medicaid participating hospitals, specifying the required elements and qualifications for a hospital's medical director, in compliance with sections 9305(c) of the Omnibus Budget Reconciliation Act of 1986 and 6025 of the Omnibus Budget Reconciliation Act of 1989.
